Description

This evaluation will allow participants to evaluate your program. Instructors should photocopy this evaluation form and hand them out at the end of the workshop(s) to gain valuable feedback from the participants. Prior to the workshop, programs should decide whether or not to require participants to include their contact information (name, address, etc.). This information will help programs to follow-up, but it might affect the level of detail and honesty of participants' answers. This indicator has been identified by PerformWell as a suitable indicator for measuring whether a workshop on financial education is meeting client needs.
